In an effort to save you time in our office, we welcome you to complete your patient history forms online prior to your appointment. Let us know what questions, concerns and goals you have regarding your eye health on the form. Just click the link below to be directed to our demographic and history questionnaire. When you are finished, hit submit and you’re done!

Once you click the button, new patients  please click the ‘New Patient Page’ link to proceed.  If you are an existing patient, please enter your passcode and click ‘Submit’. Please contact our office if you do not know your passcode.

If you cannot complete the online history form, please arrive 20 minutes prior to your appointment to fill out the questionnaire in our office. We look forward to meeting you!

If you would like to request a copy of your records,  please print and complete the below forms and email or fax it to us.  As a general estimate, it can take about two (2) business days for active patient’s records and may be longer for older records.

Insurance Plans

At Princeton Family Eye Care we personally work with you and your insurance providers to make sure you get the full benefit of your coverage. We accept both vision and medical insurance which are very different in terms of the services they cover.

Vision insurance (e.g. VSP, Spectera) is a wellness benefit designed to reduce the cost of routine annual eye exams and prescription glasses and contact lenses.

Medical insurance (e.g. BCBS, Aetna, Medicare) is designed to help cover the cost of a medical problem, including one that affects your eyes. Examples include dry or itchy eyes, red eyes, eye pain, floaters, headaches and diabetes. Some medical insurance plans ALSO cover routine eye exams for patients who need a prescription for glasses or contact lenses and are otherwise healthy.

We are continually in the process adding our office to more insurance plans.  If you have questions, please call and our knowledgeable staff would be happy to assist you.
